Continuum provides population health, practice transformation and applied analytics resources to help physicians transition to value-based care. It currently serves more than 1,500 providers. With its investment in PIC, Continuum plans to further develop its population health services in New Jersey.
“Continuum is dedicated to accelerating value-based care in New Jersey, and our new relationship with PIC will help us meet that goal,” Don McDaniel, Continuum CEO, said in a press release. “It’s critical that providers think about managing the total cost of care — for their practice, patients, and payers. We are thrilled to align ourselves more closely with PIC and collaborate with them to sustain and grow a vibrant provider community.”
