Village Health Partners will use clinical care coordinators to help patients with chronic conditions schedule appointments, provide health education and generally work through the healthcare system. The care coordinators are employed by Village Health and are aligned with Cigna case managers to ensure collaboration.
The initiative was effective Jan. 1, 2013.
