The partnership will focus on providing effective, managed care, especially to patients with chronic illnesses. The 11 locations of Cape Regional Physician Associates have staff dedicated to talk to patients about their care plan and to provide support, according to the news release. Offices will also have patient care coordinators available, who spend extra time coordinating care for chronically ill patients.
“The PCMH is a patient-centered, physician-led, highly interactive approach to the most up-to-date clinical treatment plans,” William Hofmann, DO, outpatient medical director of CRPA, said in the release. “It is supported by a system-wide electronic health record.”
