The ACO grants are being made as part of Blue Shield’s pledge that it would return money to its customers when its net income exceeds 2 percent of revenue. Grantees include Doctors Medical Center of Modesto ($650,000), which plans to deliver a post-acute discharge program that aims to reduce post-discharge readmissions and emergency room visits, improve primary care follow-up and reduce overall length of stay.
