The partnership aims to help clinicians ensure that a patient’s foundational needs such as food, clean water and housing are met, according to an Oct. 13 news release from PointClickCare. Research from the National Academy of Medicine shows that social determinants of health make up more than 80 percent of a patient’s health outcomes.
