In PCMH models, physicians lead care teams and track clinical and nonclinical factors that may affect patients’ healthcare. BCBSM reported 1,422 designated PCMH in July 2014.
A three-year longitudinal study of more than 2,000 practices in Michigan was published last month in JAMA Internal Medicine, and showed cancer screening rates improved across all socioeconomic groups. The study also showed a decreased disparity in screening rates between high and low socioeconomic groups when the model of care was fully implemented.
