Heritage California ACO, Fresenius Medical Partner for ESRD Care

Heritage California ACO, a Pioneer accountable care organization based in Northridge, Calif., has partnered with Fresenius Medical Care, the nation’s largest provider of kidney care services based in Waltham, Mass., to provide integrated care management of the ACO’s end stage renal disease patients.

The partnership aims to improve ESRD patients’ lives and health outcomes while lowering the cost of treatment.

“We are excited to be partnering with Fresenius Medical Care…to prove that through coordinating care we can improve the lives of individuals suffering from end stage renal disease,” Richard Merkin, MD, Heritage’s president and CEO, said in a news release.
