CMS Selects 17 Sites for Community-Based Care Transitions Program

CMS has selected 17 sites to participate in the Community-based Care Transitions Program, a five-year program created under the Patient Protection and Affordable Care Act to test models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries.

The 17 sites join 30 existing participants of the program, bringing the program’s total number of acute-care hospitals partnering with community-based organizations to 200.

Under CCTP, participants sign two-year program agreements with CMS with the option to renew each year for the remaining three years, based on their success, according to CMS.
