The following is a list of hospitals with a 30-day readmission rate from heart failure of 18.6 percent or lower, according to data from CMS.

Information was pulled from Hospital Compare and is from July 2012 through June 2015, the most recent data available.

For reference, the national average heart failure readmission rate is 21.9 percent.
