Salt Lake City-based Manor on 1st, a physician-owned and operated assisted living community, is opening Aug. 15, CBS affiliate KUTV reported Aug. 13.
The facility’s care model focuses on proactive chronic disease management and coordinated transitions between assisted living, home health and hospice care. It combines personalized assisted living with physician-led clinical oversight.
The facility was founded by Mark Cacciamani, MD, a hospice and palliative care specialist.
