Mt. Hood Senior Living is a 50-bed facility that the Oregon Department of Human Services alleges was a “serious threat to resident health and safety.”
The order issued to the facility ownership listed seven violations, including failure to provide a sanitary kitchen, failure to provide properly trained staff and a failure to safely administer medication. The document also references a resident death in December; an 81-year-old resident wandered away from the facility and was found dead a day later.
“These findings pose an immediate jeopardy to residents,” the order said. Families of the residents received an email the afternoon of Jan. 26 alerting them that their loved ones were being relocated to another facility before midnight that day.
