New York City-based NYC Health + Hospitals’ Housing for Health initiative has launched a pilot program that places home health aides with medically vulnerable patients during the critical gap between housing move-in and Medicaid approval — a window that typically lasts 45 to 90 days.
The Bridge to Care program will close service gaps by providing medically vulnerable patients with personalized care to support their transition to independent living, according to a June 25 system news release. It is part of the Housing for Health initiative designed to expedite placements of unhoused patients into supportive housing. The program provides patients with the health evaluations required for approval to access supportive housing citywide, and supports patients as they transition into permanent housing.
Since launching its Housing for Health initiative — which also operates medical respite beds and ground-leases hospital land for affordable housing development — the health system has permanently housed more than 1,800 patients.
