A Warrensville Heights police report said Amber Henderson neglected to report an 84-year-old patient had gone missing for nine hours. After police had been called, the patient was found dead outside the center. The Cuyahoga County Medical Examiner’s Office determined the patient died from hypothermia, the WEWS report said.
