Chicago-based Northwestern Medicine has launched a Geriatric Emergency Department Initiative at Palos Heights, Ill.-based Northwestern Medicine Palos Hospital.
The program will deliver specialized care to patients 65 and older during and after an emergency visit, according to a May 21 news release from Northwestern Medicine.
Care teams will receive training tailored to providing care for older adults during the visit, and will partner with local fire, police and EMS teams to identify whether patients are at risk. Nurses will also provide resource packets, follow-up calls and daily check-ins after the visit.
Northwestern Medicine launched the GEDI program in 2013 at Northwestern Memorial Hospital in Chicago and aims to expand the initiative to all emergency departments in the health system.
