- Spencer Post Acute Rehabilitation Center, 82 beds in Spencer, Iowa.
- Hillside Village of De Soto Rehabilitation and Nursing Center, 49 beds in De Soto, Kansas.
- Creekview Health and Rehabilitation, 78 beds in Knoxville, Tennessee.
- Foothills Transitional Care and Rehabilitation, 135 beds in Maryville, Tennessee.
- Midlothian Healthcare Center, 120 beds in Midlothian, Texas.
- South Davis Specialty Care, 95 beds in Bountiful, Utah.
- Western Peaks Specialty Hospital, 43 beds in Bountiful, Utah.
These acquisitions bring Ensign’s portfolio to 310 healthcare operations, 29 of which are senior living operations, in 14 states.
