The Right Place, a digital health startup, provides software to help hospitals reduce administrative burden when discharging patients to skilled nursing facilities. BIDMC will use the software solution to electronically refer patients to SNFs located throughout the country.
More than 200 care facilities are currently connected to The Right Place, which tracks real-time data on how organizations respond to patient referrals, how many SNFs offered beds, how many patients SNFs accepted and patients’ length of stay. The data also helps hospitals and SNFs assess quality measures CMS added for nursing homes last July.
