The chain has 35 facilities in Illinois, according to the website of Levin and Perconti, a law firm that has represented defendants suing the chain.
One of the more significant penalties against Aperion Care by the Illinois Department of Public Health was a $50,000 fine for failing to follow a physician’s order that resulted in the death of a resident in January 2019.
Aperion Care was also fined $25,000 after staff failed to measure a patient’s blood sugar, and the patient subsequently died from an insulin spike.
