The resident, 76-year-old Phyllis Campbell, was found outside the facility around 8:30 a.m. Jan. 7, when temperatures reached below freezing around much of Ohio.
The Putnam County sheriff’s office and the Pandora Police Department are investigating the resident’s death.
No further details about Ms. Campbell’s death were released.
