The 340B Drug Pricing Program helps nearly half of U.S. hospitals stretch scarce resources to care for underserved patients. HRSA’s revised Rebate Model changes how a subset of that savings reaches covered entities: instead of an upfront discount, covered entities will pay wholesale acquisition cost, known as WAC, on drugs within Medicare’s Drug Price Negotiation Program for 2026 and 2027, then submit claims and wait to be repaid the difference. Covered entities purchased an estimated $100 billion in 340B drugs in 2025, and the drugs covered by the 340B Rebate Model Pilot make up about 5.5% of that volume, or roughly $5.5 billion in annual 340B purchases. At a typical 340B discount of around 50%, the model puts an estimated $2.75 billion in 340B savings at stake.

If a rebate submission is incomplete, delayed, or denied, 340B savings are at risk. 340B Health, the hospital trade association, has separately warned that a rebate model would force safety-net hospitals to float millions of dollars annually while awaiting reimbursement, with the burden scaling sharply for larger hospitals. Daily claim-level submission is expected to become the operational standard to keep that risk as small as possible, a cadence without precedent under the traditional 340B discount model, and will strain 340B teams already stretched thin. Covered entities that rely on manual spreadsheet-based tracking will not be able to sustain it.
