Saratoga Hospital in Saratoga Springs, N.Y., has earned Age-Friendly Pharmacy Recognition from the American Society of Consultant Pharmacists, becoming the first hospital in the nation to receive the designation, according to an Aug. 7 report by The Saratogian.
The recognition honors pharmacies that demonstrate excellence in medication management and care for older adults through evidence-based practices. Saratoga Hospital Pharmacy met standards across the 4Ms Framework — What Matters, Medication, Mentation and Mobility — as well as leadership, education and quality improvement. The 4Ms Framework is designed to help healthcare organizations align care with older adults’ priorities while optimizing medications, supporting cognitive health and preserving mobility.
To earn the designation, at least 50% of the department’s pharmacists completed specialized education and training to receive individual Age-Friendly Pharmacist recognition. The recognition program was developed by the American Society of Consultant Pharmacists in partnership with the John A. Hartford Foundation and the Peter Lamy Center on Drug Therapy and Aging.
