Atlantic Health partnered with FeelBetter, which offers polypharmacy patient management technology, according to an April 9 news release. The health system’s new tool will find patients at high risk of hospitalization, recommend edits to medication regimens and other clinical interventions, and “monitor care journeys at the individual and population health levels,” the release said.

Every year, substandard medication management leads to 275,000 deaths and more than $528 billion in avoidable costs in the U.S., according to research

The health system provides care for more than half of New Jersey and parts of New York and Pennsylvania, and FeelBetter’s platform will be available for more than 520,000 beneficiaries via Atlantic Health’s three Accountable Care Organizations.

At the Becker's 11th Annual IT + Revenue Cycle Conference: The Future of AI & Digital Health, taking place September 14–17 in Chicago, healthcare executives and digital leaders from across the country will come together to explore how AI, interoperability, cybersecurity, and revenue cycle innovation are transforming care delivery, strengthening financial performance, and driving the next era of digital health.
