ASHP honors Michigan Medicine exec for pharmacy integration

Michigan Medicine’s chief population health officer, Hae Mi Choe, PharmD, recently won a leadership award from the American Society of Health-System Pharmacists. 

Dr. Choe received the ASHP board’s Distinguished Leadership Award for her work in catalyzing “pharmacists’ involvement in accountable care organizations, patient-centered medical homes, specialty pharmacy, and other organized care settings,” the ASHP said in a July 2 news release. 

In addition to chief population health officer, Dr. Choe serves as the associate dean for pharmacy innovations and partnerships at Ann Arbor-based Michigan Medicine, CEO of the system’s ACO, and executive director of the Michigan Institute for Care Management & Transformation with Blue Cross Blue Shield of Michigan. 

She has launched several initiatives to integrate pharmacists into clinical care teams, including a group practice model and a hypertension care model for clinical pharmacists, as well as a reimbursement process for clinical pharmacy services.
