Under the pilot program, eligible Medicare Advantage members with Type 2 diabetes can use the Dexcom Mobile Continuous Glucose Monitoring System to monitor blood glucose levels 24/7. The wearable device comprises a sensor, worn on the abdomen, that reads glucose levels. The reading is sent via transmitter to a smartphone, which processes and displays the data every five minutes to help members understand relationships between eating, exercise and blood sugar levels.
Eligible participants will also receive diabetes coaching and an activity tracker to help members act on the data gathered by the wearable system.
