Americans ranked Blue Cross Blue Shield as the most relevant health plan provider, according to Prophet's latest brand relevance index.

To determine how brands ranked, global consultancy firm Prophet surveyed 12,694 U.S. consumers in June about more than 299 brands in nearly 40 industries. The firm surveyed consumers on multiple factors, including how much they depend on brands, the brands they consider innovative and how much they're inspired by brands.

Here's how the top six U.S. health insurers stacked up:

1. Blue Cross Blue Shield
2. Humana
3. Anthem
4. UnitedHealthcare
5. MetLife
6. Cigna
