Survey: 35% of seniors have no one to help coordinate their care

In a survey of more than 1,000 seniors conducted by Harris Poll, 34 percent of respondents said they rely on family members to coordinate care across providers and 35 percent reported having no assistance navigating their care.

Additionally, 64 percent of seniors reported seeing three or more healthcare providers in the last year. These numbers and the multitude of complex, chronic conditions that affect seniors suggest a lapse in care regarding the engagement of senior patients.

“Navigating the healthcare system is inherently challenging and is even more perplexing for seniors, as many suffer from chronic diseases like heart disease, hypertension and diabetes, and are at high risk for complications,” said Sachin H. Jain, MD, president of CareMore Health System. “It’s important that we engage patients by providing access to a comprehensive healthcare team and services, to enable access to optimal care and coordination. Successful care delivery models of the future will harness the power of teamwork to meet the medical, psychological, social and personal health needs of the patients they serve.”
