VHA to Support Patient-Centered Medical Neighborhood Demo

VHA and two other organizations are starting in a patient-centered medical neighborhood demonstration project, a newer concept designed to improve primary health from a community-level perspective.

VHA, along with TransforMED and Phytel, are collaborating in the project, which is funded by a three-year, $21 million cooperative agreement awarded by the Center for Medicare and Medicaid Innovation. In total, 157,000 Medicare and Medicaid beneficiaries across 15 health systems will participate in the demonstration.

The PCMN project aims to decrease healthcare costs by $49.5 million within the 15 communities and achieve a 25 percent improvement in the patient experience, among other objectives.
