The Healthcare Association of New York State submitted comments on New York State's Health Home Application to Serve Children, encouraging the state's ideas while also offering recommendations and concerns.
The draft application from the New York Department of Health suggests tailoring the existing Health Home model specifically toward children to recognize and address the differences in approaching care management and planning between children and adults.
HANYS expressed concerns about two elements. HANYS mentioned "the inadequacy of care management rates to meet the costs of establishing and maintaining a Health Home, and patient assignment to a Health Home." The letter asks the state to recognize past challenges and then make extra efforts to minimize them.
