State health officials said Thomas Arthur Green was found unresponsive in his Anoka room Oct. 27 and died a week later at a hospital, according to the report.
“We are all saddened by this tragic loss,” Human Services Commissioner Emily Johnson Piper said in a Wednesday statement. Ms. Piper said the department is leading multiple reviews of Anoka’s 110-bed facility.
The treatment center has been under federal review for most of the past year following discovered lapses in care, according to Star Tribune. CMS said the facility’s Medicare and Medicaid funding is in “immediate jeopardy” and will oversee Anoka through May 2018. The state has brought on more staff and continued to have a consultant suggest changes.
