According to court documents, John Balko, MD, of SHA allegedly submitted false claims to the Medicare program that were medically unnecessary, unauthorized, not requested by patients and were unsupported by patient medical records.. The claims consisted of services provided to nursing home residents, including earwax removal, podiatry and certain evaluation and management services.
Office of the Inspector General conducted the investigation.
