CMS Creates Medicare Chronic Conditions Dashboard

CMS announced a new Medicare Chronic Conditions Dashboard to provide researchers, physicians, public health professionals and policymakers data on the geographic spread, required services and costs for beneficiaries with multiple chronic conditions.

The dashboard, part of HHS’ Initiative on Multiple Chronic Conditions, is designed to help users find, analyze and apply summarized data from CMS’ Chronic Conditions Data Warehouse to improve care and reduce costs for patients with multiple chronic conditions.

In addition, the dashboard will help CMS and its stakeholders identify communities that have successfully managed beneficiaries with multiple chronic conditions, which can guide programs, policies and best practices for the whole healthcare system, according to the release.
