Prospect Medical, which has 16 hospitals across four states, will join a Signify-enabled, data analytics-powered ACO to coordinate care for its fee-for-service Medicare patients in Connecticut.

“Through our work together, Prospect’s Medicare patients will receive needed increased access to preventative and in-home services that will help improve their health outcomes while creating a better overall experience for both patients and their care teams,” said Tim Gronniger, chief of value-based care solutions for Signify, in a joint Jan. 11 news release.

CVS Health has agreed to buy Signify for $8 billion pending regulatory approval.
