The E3 Patient Safety Grant Program will award three types of projects this year:
- Projects that improve medication safety as patients transition from the hospital to home.
- Projects that use technology, including the use of smartphones, apps, tablets, remote monitoring and telehealth through consumer devices, to increase access to care and engage patients.
- Projects that help spread previously successful work.
