Through the partnership, city employees, spouses and their dependents age 2 and older may receive healthcare services at CHI Memorial Family Practice Associates – Chickamauga (Ga.) and Professional Park Associates Convenient Care – LaFayette, effective Jan. 2. Previously, Walker County Commissioner Shannon Whitfield’s office offered city employees healthcare services, according to a Times Free Press report.
Services available to employees at both locations include primary care, urgent care and chronic condition management including needed laboratory services. Additional services available to employees at the LaFayette location include digital X-ray and pharmacy.
“For those with chronic conditions, CHI Memorial will assist in the management of their condition through its nurse-led health coaching program. As well, employees, spouses and dependents in need of care from a specialist will benefit from the care coordination that occurs within the CHI Memorial network of providers,” the hospital added. “CHI Memorial will also provide educational information on disease prevention and management, as well as help people with smoking cessation.”
