The facilities include Agnesian HealthCare, a three-hospital system in Fond du Lac, Wis., and Monroe (Wis.) Clinic. The transaction will also add eight post-acute facilities into the SSM Health family.

Both health facilities and their affiliates will adopt the SSM Health brand while also keeping their initial branding. Officials expect to complete the transition by the end of 2018.

The CSA initiated the change in sponsorship, citing changes in the state’s payer and provider markets. CSA and SSM Health signed a letter of intent for the transfer last July.
