MSU HealthTeam and its affiliated clinics join the roughly 2,900 primary care practices involved in the partnership.
Comprehensive Primary Care Plus, a five-year, primary care medical home model, was created through a collaboration between CMS and 54 state Medicaid agencies, commercial health plans, self-insured businesses and primary care providers across the country. The model launched in January 2017.
