Alameda Health System, Conifer Health partner on management solutions

Oakland, Calif.-based Alameda Health System is partnering with Frisco, Texas-based Conifer Health Solutions to provide management and consulting services to better serve the residents of Alameda County.

Under the agreement, Conifer Health will provide managed care support services, financial risk management, population health management and nurse consulting services for Alameda Health System’s HealthPAC program. HealthPAC is a comprehensive health access program that oversees 33,000 Alameda County residents.

Conifer will also provide consulting services to help AHS improve its ability to provide quality care to high-risk populations within the community.

The financial terms of the transaction, if any, were not disclosed.
