Seventy-eight percent of study respondents said their health plans were either participating or preparing to participate in an ACO and/or patient-centered medical home. The remaining 22 percent of respondents said they plan on participating in the future.

Also, almost 90 percent of respondents indicated that care management is an important part of their health plan’s overall strategy.

The survey participants were health plan executives who represented organizations that covered 200,000 to 900,000 lives.
