Community Health Connections’ New $20M Center Based on Medical Home Model

Fitchburg, Mass.-based Community Health Connections is building a new $20 million community health center based on the patient-centered medical home model, according to a Telegram & Gazette report.

An administrative office building at the back of HealthAlliance Hospital Burbank Campus in Fitchburg is being expanded and renovated to house the new center. The 69,000-square-foot facility will have twice as much space as the current center and will include medical, dental and mental health services.

The project is expected to be completed by the summer.
