Using several million dollars, BCBS of Tennessee is providing funding healthcare at the practices as well as an on-site care coordinator who reaches out to patients and community resources. Further support will be provided in the form of staff and technology.
The physicians will lead a professional healthcare team to take responsibility of the ongoing care of patients with conditions such as heart disease, chronic lung disease and diabetes.
BCBS will then reward the practices based on quality and effectiveness. The CDC estimates that chronic conditions account for 75 percent of the nation’s total healthcare costs.
