Since the pilot project was launched, 9.25 percent of patients in the study were readmitted to the hospital within 30 days compared to 16.5 percent who were readmitted the year prior. The program was in conjunction with Minnesota health plan UCare, a Medicare Advantage insurer.
A nurse case manager was assigned to help patients with practical problems and UCare offered a $50 bonus to physicians who scheduled patient follow-up visits within five business days.
