First reported by Crain’s, the health system submitted a request for proposals from interested organizations on May 20. CCHHS hopes expanding into nursing homes will help better address its patients’ needs, improve continuity of care after patients leave the hospital and decrease readmissions, according to the RFP.

The safety-net health system provides services across the continuum of care, including specialty and primary care, emergency, acute, outpatient, rehabilitation and preventative care. Adding nursing homes to its portfolio would extend this continuum even further.

CCHHS plans to award a three-year contract with the option to extend it for up to two years. Proposals are due July 19.
