At the forum, CMS officials will provide information on the Affordable Care Act-authorized program. The program — which is open to hospitals with 30-day readmission rates in the top quartile in their state for heart attacks, heart failure and pneumonia — will provide $500 million over five years to hospitals to improve care transitions for Medicare enrollees at high risk for readmission, according to the report.
The forum will provide an overview of the Partnerships for Patients Initiative, the role of care transitions in improving patient safety and answers to frequently asked questions.
