Valhalla, N.Y.-based Westchester Medical Center Health Network has launched a partnership with insurer MVP Health Care aimed at improving care coordination for patients during hospitalization and after discharge.
Under the arrangement, MVP clinical care managers will work alongside Westchester Medical Center care teams in the hospital and continue supporting members as they transition home and navigate follow-up care and services, according to a June 3 news release from the health system.
The organizations said the model is designed to reduce gaps between hospital and home by aligning care plans with patients’ clinical needs and insurance coverage. Research cited in the release found patients who felt uninvolved in care decisions were 79% more likely to be readmitted, while those who did not understand discharge instructions were nearly twice as likely to return to the hospital.
The partnership also expands access to WMCHealth specialty services for MVP members across the Hudson Valley through initiatives including expanded pathways to advanced care and virtual specialty consultations. The organizations plan to build on the partnership by expanding it across additional service lines, specialties and communities, the release said.
