Andover, Mass.-based Covenant Health has appointed Kim Baker as vice president of Covenant Health and president of its post-acute care ministry, according to a May 1 health system news release published by The Boston Pilot.
In the role, Ms. Baker will oversee the system’s skilled nursing, long-term care and assisted living services across Covenant Health’s post-acute care operations. She will also work to improve care transitions between hospitals and post-acute settings.
Ms. Baker most recently served as senior vice president at Trinity Health New York, where she oversaw a multisite hospital portfolio including St. Peter’s Hospital in Albany, N.Y., and Samaritan Hospital in Troy, N.Y. She was responsible for strategic planning, quality improvement, physician engagement and operational performance across the network, which serves hundreds of thousands of patients annually, the release said.
Covenant Health operates hospitals, skilled nursing and rehabilitation centers, assisted living facilities and independent senior residences across New England and Pennsylvania.
