The payer learned on Nov. 3 that a vendor’s computer coding error caused denial letters to be sent to the wrong members, according to the report.
Breached information included patient names, addresses, member ID numbers and general descriptions of the procedure. No Social Security numbers or financial information were compromised, according to the report.
