University of Maryland Medical System deploys analytics solution to reduce readmissions

Baltimore-based University of Maryland Medical System partnered with Real Time Medical Systems, a healthcare interventional analytics company.

UMMS hospitals will implement Real Time’s ProACT HS program that enables collection and analysis of patient data in real time after the patient has been discharged to a nursing home. The hospitals and skilled nursing facilities can share data and outcomes, with the aim of avoiding unnecessary readmissions.

“ProACT HS pulls data from the EHR of the nursing home patient and allows immediate interventions in care, while supplying visibility of outcomes to the health system partners,” said Scott Rifkin, MD, founder and executive chairman of Real Time Medical Systems.

Real Time Medical Systems has 350-plus health system and skilled nursing facility clients.
