Hartford (Conn.) HealthCare is partnering with Cadence to launch a remote care program for older adults with chronic conditions using AI-supported monitoring and clinical support to extend care beyond traditional visits.
The program, Hartford HealthCare Remote Care, will integrate Cadence’s Clinical Intelligence platform to provide in-home vital sign monitoring, proactive clinical outreach and personalized coaching for patients with conditions such as hypertension, diabetes and heart failure, according to a May 5 news release.
Care teams will use supervised AI to review daily patient data from connected devices and generate guideline-based recommendations, which clinicians will then review and implement within Hartford HealthCare’s existing care model. The approach is designed to enable earlier intervention between appointments and improve disease management.
Hartford HealthCare said the model could accelerate care adjustments, with some heart failure patients able to receive medication changes in days rather than weeks and help reduce hospitalizations.
The program will operate through Hartford HealthCare Medical Group using shared clinical protocols, with all AI-supported recommendations remaining clinician-supervised to ensure safety and continuity, according to the news release.
