Cleveland Clinic is expanding its hospital-at-home model to Northeast Ohio, offering some patients the option to receive acute care at home instead of a traditional inpatient stay.
The health system’s Hospital Care at Home program is launching on Cleveland’s west side and will initially serve patients treated at Fairview Hospital and Avon Hospital. The program provides hospital-level care at home for patients who meet certain clinical criteria and live within a 25-mile radius of those facilities.
Eligible patients must first be evaluated in the emergency department or admitted to a hospital unit before being approved for at-home hospitalization. Conditions that may be treated through the program include chronic obstructive pulmonary disease, sepsis, cellulitis, pneumonia, asthma and select postoperative colorectal procedures, according to a March 25 news release.
Cleveland Clinic previously launched the Hospital Care at Home program in Florida in 2023. It has since treated more than 4,000 patients and reduced hospital readmissions.
