Anthony Coletta, MD, CEO of Tandigm Health, said in a statement that his company’s new relationship with athenahealth and the implementation of its population health management tool “will better equip Tandigm network physicians to provide proactive, coordinated care to their patients.”
Tandigm Health’s network of primary care physicians provide care to more than 100,000 patients. Through the collaboration with athenahealth, Tandigm will utilize athenahealth’s population health management tool for care management as well as analytics, risk stratification and quality management, according to a news release.
