Southcoast Health debuts post-discharge app

New Bedford, Mass.-based Southcoast Health is deploying Health Helm’s patient coach mobile application to help patients with postsurgical recovery, according to a Cape Cod Today report.

The Trusted Patient Coach Mobile App allows patients and physicians to connect at any time. It offers a 24-hour, two-way connection, enabling providers to monitor patients and ask questions as well as giving patients or a designated family member access to providers. The app is password-protected and HIPAA-compliant.

The three-hospital system that offers care for 724,000 residents in 33 communities hopes the app will reduce emergency department visits post-discharge as well as improve outcomes.
